MSF is an independent humanitarian medical organization providing emergency medical aid to populations in need. MSF Somalia mission is seeking a qualified Doctor Anaesthetist Consultant as per below TOR:

Project Background

The anesthetist is responsible for delivering anaesthesia for medical and surgical procedures. The Anaesthetist cares for a patient before, during and after a medical procedure or surgery by performing a patient assessment, preparing the patient for anaesthesia, administering and maintaining the anaesthesia to ensure proper sedation and pain management, overseeing patient recovery from anaesthesia and caring for the patient’s immediate post-operative needs.

General Objectives

To ensure the safety and well-being of patients undergoing anaesthesia and surgery. This includes conducting preoperative assessments, optimizing patient health status and administering anaesthesia in a safe and effective manner.

To provide anaesthesia services to patients undergoing surgical, obstetric, or other medical procedures. This involves selecting appropriate anaesthetic techniques, administering anaesthesia medications and monitoring patients throughout the perioperative period.

To optimize perioperative care by collaborating with surgeons, doctors, nurses and other health care professionals to develop comprehensive care plan tailored for individual patient needs.

To manage anaesthesia related complications including managing airways, hemodynamic instability, allergic reactions, or other critical events.

Activities

  1. Train our team on Patient Care:

Prepare preoperative assessments including health histories, physical examinations and tests to determine if giving anaesthesia to the patient would be fine or not.

Make sure that the patients are prepared well for the surgery both physically as well as mentally.

Ensure transfer of patients to the recovery room, and full recovery of the patient’s consciousness and setting analgesic protocol.

Advise on Postoperative care in both Intensive Care Unit and wards with the surgeon, including fluid management, oxygenation monitoring and airway care and chest physiotherapy and mobilization, with special focus on patient’s pain relief during the recovering process.

Initiate and maintain appropriate intra-operative anaesthetic care. Ensure that the patients are given appropriate dosage of anaesthesia and also maintained while the surgery is going on.

Monitor the patients for a while after the anaesthesia is given. This is to check if the anaesthesia goes in properly or reacts in a different manner.

Initiates remedial measures to prevent surgical shock or other adverse conditions.

Provide information related to anaesthesia to the patients. He/she needs to calm the patients’ anxiety and educate him/ her about the after effects of anaesthesia.

Document and archive patients’ reports; preoperative assessments, intra and postoperative anaesthesia procedures.

Complete postoperative evaluations in accordance with department policies.

Ensure patients are treated adhering to MSF protocols.

  1. Team Management:

Implement and supervise hygiene rules, procedures and MSF protocols in OT/recovery, anesthesia material (decontamination, sterilization, storage conditions, etc.) to ensure the safety and protection of patients and staff.

Supervise the OT pharmacy (drugs orders, inventories, expired drugs) and ensure OT register, consumptions, empty phials of narcotics, ensuring adequate stock is maintained.

Organize the tasks of personnel under his/her responsibility, and participate and carry-out other team management activities (recruitment, training, performance evaluation, motivation, internal/external communication, etc.), in order to ensure having the appropriate team in terms of size, capabilities and skills.

Collaborate with other departments in the project (responding to accident emergency department requests, intervening in support of other services when needed, etc.), and participate in any emergency activity or exploratory mission, in order to support the overall MSF project with his/her expertise and capabilities.

Ensure proper fulfillment of administrative procedures and documents (fill in of patients files, forms, statistics, data base, etc.), and elaborate regular reporting about results, proceedings and problematic situations and cases, in order to have updated and correct information about the day-to-day activity in the project and support decision-taking.

Perform any other duty assigned by line manager.

Face to face interactions with the medical doctors. Case presentation and discussions with the doctors.

Conduct preoperative assessment including previous surgeries, medical conditions, allergies and medication use.

Develop an individualize anaesthesia plan and anticipate anaesthetic requirement Briefing and debriefing PMR and Medical coordinators

Expected Out comes

Build capacities of the anesthetist in hospitals supported by MSF

To improve patient safety by implementing evidence-based practices and MSF Protocols for anaesthesia management.

To enhance interdisciplinary collaboration including surgeons, doctors, nurses and other specialists. This will improve teamwork, communication and coordination leading to patient-centered care delivery.

Education and training opportunities for other healthcare providers to enhance their skills and competencies. This include continuing medical education (CME) activities, simulation trainings and professional development for the latest in anaesthesia practices.

Patient-Centered Care by prioritizing patient safety, comfort and satisfaction through peri-operative journey

Requirements for the consultant:

Doctor Anesthetist diploma recognized by the Somalia MoH.

Duration of consultancy

6 Months (3moths in Baidoa and 3 Months in Mudug).
