Job Title: HMIS (Health Management Information System) Assistant (1 Position)

Location: Mogadishu-Garasbaley

Job Summary:

The HMIS Assistant creates new medical records and retrieves existing medical records by gathering appropriate record folders and contents; assigning and recording new record numbers; verifying existing record numbers; inputting and recording locations to computer; delivering records.

Report to: MEAL Officer

Key Area of responsibility:

  1. Maintains master patient index by completing assigned portion of daily audit trail; corrects and communicates problems according to established procedures.
  2. Initiates the medical record by creating and processing the patient care record folder.
  3. Maintains record availability by processing charts into the department; using chart mark-off procedures; facilitating chart location activities.
  4. Retrieves medical records by following chart-out procedures; documenting reasons charts cannot be retrieved for statistical and follow-up purposes.
  5. Delivers charts to assigned areas of the hospital by following established routing procedures.
  6. Keeps health care providers informed by communicating availability or unavailability of the record.
  7. Maintains quality results by following hospital standards.
  8. Maintains continuity of work operations by documenting and communicating actions, irregularities, and continuing needs.
  9. finding and checking patient records
  10. creating new records and updating existing ones on the computer system
  11. Forwarding test samples to laboratories
  12. Inputting details from paper records onto a computer system
  13. Maintain patient confidence by keeping patient records information confidential.

  1. Serves and protects the hospital community by adhering to professional standards, hospital policies and procedures and local requirements
  2. Storing test results and letters with the right patient records
  3. Maintain a relational database system for the clinic records i.e pharmacy, laboratory, consultations, asset register Generate identification cards for target groups
  4. Recording patient admissions, transfers, discharges and deaths
  5. Filing records
  6. Recording illnesses and treatments using a system of codes, known as clinical coding
  7. Collecting statistics such as number of admissions, discharges and waiting lists

Qualifications and Experience

  • Bachelor degree or Health information Management system. Or relevant certificate o Has experience in Health basic understanding

o Has at least one year experience in HMIS o Worked in HIMS or data management

o  Has understanding on statistics

o  Has understanding of filling system in electronic
