Since January 2019, MSF is supporting the National Tuberculosis Program in Somaliland and started to work in Hargeisa TB Hospital, specifically in the Drug Resistance Tuberculosis centre and supporting 3 Regional Hospitals in Tuberculosis. The aim is to reduce the morbidity, mortality and burden of TB and more specifically MDR-TB with below objectives: o To improve the quality of DR-TB diagnosis in MSF supported facilities (Hargeisa DR-TB hospital and Regional TB centres (Boroma, Berbera and Burco) o To improve Infection Prevention and Control in Hargeisa TB hospital and the Regional TB centres supported by MSF (Boroma, Berbera and Burco) o To improve MDR-TB patient care and treatment Outcomes of Hargeisa MDR-TB centre cohort o To increase DR-TB case finding using different models to increase access to TB screening and diagnosis |

Nursing Team Supervisor – Outreach
Workplace : Hargeisa & Regions
Type of contract : Fixed Term Contract with Three-month probation
Start date : 1st January 2022
Main purpose of the position
| The main purpose of the MSF regional TB nurse is to ensure the implementation of activities related to project set objectives. The Regional TB Nurse will be the contact point for MSF teams in Hargeisa and serve as liaison between the MSF TB project team and the MoHD staff in the designated TB facility. He/She will be actively involved in MDR-TB case identification and referrals to the MDR-TB center in Hargeisa as well as ensuring strict compliance to MSF standard IPC protocols, MDR-TB treatment adherence and case follow up. The Nurse focal point is also tasked with the responsibility of active surveillance |
Main responsibilities
- General role:
- With support from the NTP, MoHD regional medical officer/hospital director; coordinate the referral of MDR-TB patients from TB clinics to Hargeisa MDR-TB center.
- Support the designated regional TB Hospital with implementation of IPC activities as per MSF protocols
- Collaborate with the MOHD Outreach team for DR-TB contact tracing, follow up of MDR-TB cases, data collection analysis and report
- Support the designated regional TB Hospital with implementation of IPC activities as per MSF protocols
- Collaborate with the MOHD Outreach team in TB outreach (TB awareness, sputum collection and G-xpart testing activities) also follow up of result, referrals of MDR-TB cases, data collection analysis and report
- Collaborate with the TB lab in the designated hospital to ensure proper implementation of Gene-Xpert based testing for TB patient diagnosis and report any challenges to MSF nursing activity manager.
- Specific role:
Early case detection:
- Follow up of MoHD Outreach teams to ensure tracing of MDR-TB case contacts.
- Support the MoHD Lab technician to ensure collection of quality sputum and transportation to the lab.
- Support the MoHD lab technician and lab responsible In-Patient education and guidance on proper sputum sample production.
- Supervise triage and segregation of patients based on severity of presenting signs and symptoms.
Quality treatment:
- Ensure quality DOT for MDR-TB.
- Ensure proper follow up of MDR-TB patients for adherence to treatment and early detection of side effects with referrals for proper management.
- Ensure regular follow up visits and assessments for all MDR-TB cases.
- Provide patient education related to TB infection prevention and control, adherence to treatment, how to manage treatment challenges and side effects as well as need for HIV treatment if/when necessary.
- Ensure HIV screening thought PICT (VCT).
- Organize and attend ward round routinely with the MoH TB management team.
Infection control:
- Ensure the provision of mask to patients as part of IC in patient waiting area.
- Ensure all staff are respecting infection control protocols according to MSF/WHO standards
- Support the MoHD TB Responsible in definition and implementation of patient flow pathways as well as strict control of visitors (relatives and other non-TB hospital staff) to the TB unit.
- Support the MoHD in establishing an infection control committee and suggest improvements.
Pharmacy:
- Ensure constant supplies of MDR-TB drugs are available for patients returning to their home communities at BMUs
Patient support:
- Ensure proper patient education and counselling before starting treatment and during follows up visits.
Outreach activities:
- Collaborate with outreach team to ensure regular visit of decentralized TB clinics and organize referrals of patients if needed.
Human resource:
- Be available on call if needed.
- Identify and organize training need for hospital and outreach staffs in the concerned region
Documentation and reporting:
- Report of activities to the NAM on weekly and monthly as required .
- Participates in developing project documents and log frame.
Others:
- Follow up MDR patient after treatment in Hargeisa DR-TB hospital as required
- Reports to the Nurse Activity Manager for any incidents or context related outbreak or epidemics
Requirements:
| Education | Essential nursing diploma for general or specialized health services |
| Desirable | Essential working experience of at least two years as TB nurse. Desirable working experience in MSF or other NGO’s in developing countries. |
| Languages | Essential, English and Somali speaker. |
| Knowledge | Computer literacy and report writing |
