Job Responsibilities

Implementation:

§   Case finding and follow up of cases provided by outreach nurses’ staff.

§   Compiling reports on screening, defaulter tracing, referrals and absentees.

§   Provide educational sessions on nutrition, proper hygiene, water and sanitation promotion at the community level.

§   Reporting

§   Liaise where necessary with community leaders to assist in community mobilization and sensitization.

§   Advocate for underserved individuals or communities to receive services and resources to address health needs

§   Collect data and relay information to stakeholders to inform programs and policies

§   Provide informal counselling, health screenings, and referrals

§   Build community capacity to address health issues

Administration:

§   Keep accurate records of villages visited and beneficiaries’ information as provided by the supervisor

§   Keep accurate records of the number of children referred to and/or rejected from nutrition or health programme

§   Keep a record of community education sessions given either one on one or in a group setting.

Coordination:

§   Liaise with the location Supervisor, mobile team, outreach nurse, and other staff to ensure maximum effectiveness of programme activities.

§   In areas where other programmes/organizations are operational, liaise and be aware of the scope of work and participate in coverage areas.
