Position title        :                HMIS Assistant (1 Position)

Reporting to         :               Hospital Admin

Location                :               Baidoa

Position Summary

The HMIS Assistant creates new medical records and retrieves existing medical records by gathering appropriate record folders and contents; assigning and recording new record numbers; verifying existing record numbers; inputting and recording locations to computer; delivering records.

JOB RESPONSIBILITIES:

  • Lead and coordinate monthly and quarterly data quality audits at facility level as well as mobile outreach team.
  • Weekly review registers together with programme team at the facilities
  • Maintains master patient index by completing assigned portion of daily audit trail; corrects and communicates problems according to established procedures.
  • Initiates the medical record by creating and processing the patient care record folder.
  • Maintains record availability by processing charts into the department; using chart mark-off procedures; facilitating chart location activities.
  • Retrieves medical records by following chart-out procedures; documenting reasons charts cannot be retrieved for statistical and follow-up purposes.
  • Delivers charts to assigned areas of the hospital by following established routing procedures.
  • Keeps health care providers informed by communicating availability or unavailability of the record.
  • Maintains quality results by following hospital standards.
  • Maintains continuity of work operations by documenting and communicating actions, irregularities, and continuing needs.

  • Inputting details from paper records onto a computer system
  • Maintain patient confidence by keeping patient records information confidential.
  • Serves and protects the hospital community by adhering to professional standards, hospital policies and procedures and local requirements
  • Storing test results and letters with the right patient records
  • Maintain a relational database system for the clinic records i.e pharmacy, laboratory, consultations, asset register.
  • Generate identification cards for target groups
  • Recording patient admissions, transfers, discharges and deaths
  • Filing records
  • Recording illnesses and treatments using a system of codes, known as clinical coding
  • Collecting statistics such as number of admissions and discharges
  • Make orders for stationery, equipment and any other office supplies.
  • At all times ensures that personal data of donors, sponsors, SOS CV beneficiaries and their families as well as SOS co-workers is handled confidentially and in accordance with prevailing SOS-Kinderdorf International data protection laws.
  • Performs other reasonable tasks as assigned from time to time

Skills and Qualifications

Qualification and Job competencies:

  • Bachelor degree or Health information Management system. Or relevant certificate o Has experience in Health basic understanding
  • Has at least one year experience in HMIS or Worked in HIMS or data management or Has understanding on statistics
  • Has understanding of filling system in electronic Excellent understanding of the local context and able to maintain good working relationships with team members.
  • Strong communication skill both writing and verbal
