INTERNAL & EXTERNAL VACANCY ADVERTISMENT
The Ministry of Health Development, Republic of Somaliland requires the following positions to work in the measles isolation, ITFC and pediatric ward s in Las Anod General Hospital; these departments are supported by Medecins Sans Frontieres (MSF) – an international, private, non-governmental, non-profit humanitarian organization. Potential applicants can be external candidates or existing employees of the Las Anod Hospital. Selected candidates would be seconded by MoHD to work in the MSF supported department for specific duration:
Job Title: Nurse – (Based in Lasanod with possible frequent travel around Sool region)
Number of positions: 4 Four
Level: 5 (on MoHD Seconded Staff daily wages Pay-scale)
Work Location: Las-Anod General Hospital, Sool region, Somaliland.
Start date: Immediately after recruitment process
Length of contract: None contracted – Daily wages position
The selected candidate will be reporting to Medical supervisor (Hierarchically) & Unit supervisor (Functionally).
Main Purpose:
- Ensure nursing care, treatment and follow-up of patients in accordance with doctors’/ consultant’s prescriptions, protocols, respect of universal hygiene standards and context.
- Assess daily changes in the condition of the patients.
- Carrying out all pharmacy related activities (stock control, station’s re-stocking, inventories, expired drugs, etc.) and other equipment of measles isolation ward, doing special follow-up of (register, consumptions, empty phials, etc.), ensuring enough stock-up and the good functioning to carry-out the medical activities. Drawing up pharmacy orders according to needs
- Improve referal pathways between peripheral facilities (MCH) by identifying critical patients from community and health care facility and linking them with LAGH
- Understand the general situation of mealses out break.
Specific Responsibilities and Tasks:
- General Knowledge of MSF and Safety at work
- Co-operation with the colleagues of own and other departments
- Is aware of activities of other departments and respect specific rules and procedures
- Comply with the health and safety rules related to the job for the sites.
- Respect medical ethics at all times
- General Responsibility
- Assist in the hygiene and minor maintenance of the ward and be part of the implementation of the waste management.
- Participate in meetings, trainings as needed and required
- Ensure quality of the sterilization process.
- Assist patient if needed with feeding, his personal hygiene, his movements and his comfort.
- Organise and carry out care and treatments according to doctor’s/ consultant’s prescription and protocols in place.
- Ensure that quality and continuity of care are respected.
- Carry out surveillance of the patient, assessment of his health state evolution, identification of emergency situation and information of the doctor.
- In case of emergency, carry out first aid care and treatment according to the protocols.
- Carry out when necessary a therapeutic education of the patient (and family) using a patient based approach.
- Ensure a relevant transfer of information to the next duty team (handover).
- Report immediately to the technical referent any problem arising in the service, especially the loss/ robbery/damage of medical equipment or medicines.
- Co-operation with the colleagues of own and other departments
- Respect medical ethics at all times.
- a) Responsibilities on admission in all wards:
- Assess the condition of the patient, and record the findings in the patient file.
- Record in the register book(s) the required information.
- Ensure the children are vaccinated in OPD (if is a paediatric patient): if not, follow up and make sure this is done. Document in the patient file, and educate the patient/caretaker about EPI.
- Give information to the patient and the caretaker about their rights and responsibilities while admitted and about the care and the treatment and its side-effects.
- Inform the patient/caretaker about the routines in the health centre, the locations and use of the water sources, use of latrines, feeding times etc
- b) Genera nursing care in the ward:
- Provide nursing care according to the doctors prescription and protocols.
- Control vital signs such as Blood Pressure, Temperature, Pulse and Respiratory rate. Analyse the results and respond accordingly.
- Correctly fill in patient files; record vital signs; findings etc;
- Assess, follow up and evaluate the condition of all admitted patients, and inform the Supervisor/Consultant or Medical Doctor of progress and problems or deterioration in the patient’s condition.
- Assist the medical doctor/consultant during the in-patient rounds.
- Assist the medical doctor/consultant when interviewing and examining patients if needed.
- Ensure that all children admitted to the wards are vaccinated according to the national schedule.
- Ensure that the in-patients receive food.
- Give general health education to patient/caretakers if needed.
- Inform the patients/caretakers about the treatment and medication given, and ensure that the message is understood.
- Ensure that the patient gets the medication prescribed.
- Follow up patients that are not present during the ward or medication rounds.
- Collect drugs in the pharmacy, if necessary.
- Prepare the trolley for the medical round.
- Distribute the medications prescribed and confirm the following; right patient, right time, right dosage, right drug; right administration route and check the expiry date of the drug.
- Participate in planning and organizing the work.
- Apply dressing according to the prescription and to protocol.
- In close collaboration with the other nurses ensure that the ward is never left unattended.
- c) General documentation in all ward:
- Write observations down, and make note if changes in patient’s condition.
- Documentation in medical documents of patient’s condition.
- Documentation in medical treatment cards of treatment given.
- Report daily activities & update medical consumed supplies according to the number of patients.
- Report admissions, defaulters and deaths.
- Written and oral handover report to colleagues about patient’s condition and prescribed treatment.
- Ensure, together with supervisor, that the register books (both TFC and measles isolation ward) are updated daily.
- d) Genera documentation in all ward:
- Together with the supervisor ensure the white board are updated with numbers of patients, and that the information needed for correct food supply is updated.
- Follow the responsibility for the day as directed by supervisor. Help the others when you are finished with your duties.
- Beginning and end of each shift, find out how many beds you have free to make ready for new admission
- If requested by the MD/consultant ensure that lab tests are performed and result is documented in the patients file.
- Ensure hygiene in pharmacy management and drug handling .
- Prepare disinfecting solution when needed.
- Responsible for ensuring proper hygiene conditions in the ward (good ventilation, incoming sunlight, etc.). Educate patients and caretakers to use the latrines, potties, etc.
- You must ensure you dispose of waste according to the system in place in each ward: correct use of sharps container, buckets etc (waste segregation).
- e) General on discharge:
- Ensure that the patient are well informed about their disease conditions, what treatment has been given and ,if possible, how the disease can be prevented in the future.
- Document the patient as discharged from the register book and remove the patient card and file it.
- Fill out and give the patient/caretaker referral letter if requested by MD/consultant
- If necessary inform that the patient must come back for EPI.
- Specific tasks
When working in isolation ward:
- Assess the status of the child level of consciousness.
- Helps prevent and identify the spread of infectious agents like bacteria and viruses in a healthcare environment.
- Support the child in controlling fever.
- Administer treatment according to the treatment sheet.
- Examine the child for complication such as conjunctivitis, larygeo trancheo bronchitis.
- Refer all children with moderate acute malnutrition to ITFC or the nearest MOHD MCH facilities.
- Keep proper records and prepare daily, weekly and monthly updates/report with the supervisor.
- Share information on arrival of new mealses cases including origin, reason for displacement/arrival.
- Respect the admission and discharge criteria.
- Additional tasks
- Be flexible and be able to give assistance with different activities not included in this primary job description.
